Background – What is a service delivery plan?

A service delivery plan describes the services that you offer and how they are delivered. This plan helps you manage your service, and ensures you are meeting the needs of your local community. Every Multi-Purpose Service should have a plan and a process for reviewing and updating it regularly.p>

The service delivery plan describes the services that you will offer and the levels at which they will be provided.

  1. - The Department of Health uses this information to determine whether an MPS should be established.
  2. – It helps you manage your service
  3. - The 3-year term generally covers the entire period

The planning framework for your organisation, which is managed by the state or territory government, should detail who creates it and with whom.

You must consider the needs of your community when developing your service delivery strategy. It is important to demonstrate that:

  1. - The location is ideal
  2. - You have consulted the community
  3. - The community requires a MPS
  4. - You're prioritizing your priorities
  5. - You're making the most of your resources
  6. - The service is viable and practical.
  7. - You are working in partnership with other aged and health care providers in your locality

To make the most of information already collected, work closely with your Primary Health Network. This prevents clients from receiving duplicate care and ensures that they receive the best possible care at the right place and time.

Aims and Objectives of this course

This course Facilitate individual service planning and delivery (based on CHCCCS006) will help you earn the skills necessary to conduct interpersonal exchanges in a manner that develops and maintains trust and goodwill and to maintain confidentiality and privacy of the person within organisation policy and protocols.

Our Facilitate individual service planning and delivery course is based on the unit of competency of CHCCCS006. Whilst this course is not nationally recognised, it follows the learning outcomes and elements associated with that unit. The course contains

  1. Narrated e-learning learning resources online
  2. Online Assessment tasks
  3. Ability to download your Certificate of Completion once assessment tasks are completed

What will you learn by completing the training course?

This course will include an understanding of awareness of the relevant legislation and yourresponsibilities in facilitating individual service planning and delivery.

Learning outcomes include:

  1. Recognise and respect diverse and multi-faceted needs of the individual and collaborate with other service providers as needed
  2. Provide clear and current information about service delivery and support the interests, rights and decision-making of the person in all dealings
  3. Determine the physical and psychological factors relevant to the person’s life stage that will influence service delivery
  4. Explain the purpose of the planning process and discuss different service options with the person
  5. Work with the person to determine readiness for the development of an individualised plan and select most appropriate service option
  6. Determine who needs to be included in the planning process and organise practicalities
  7. Collate and prepare information and distribute to relevant stakeholders as required
  8. Liaise with the assessor of the person’s requirements prior to the planning session as required
  9. Respect the person’s perspective, foster their strengths and capacities and promote their participation
  10. Follow process requirements of service planning tools
  11. Identify and confirm key aspects of individualised service delivery by working collaboratively with the person and other stakeholders to establish goals
  12. Consider interrelated needs of the person and plan an integrated approach to service delivery
  13. Conduct risk assessment specific to the person’s circumstances, discuss with person and work collaboratively to minimise risk
  14. Manage any conflict or differences with regard for the person’s perspective
  15. Consult with relevant people to assess the quality of, and satisfaction with, service
  16. Address and report any problems with the quality of, or satisfaction with, service delivery within organisation procedures
  17. Work with the person and relevant others to identify and respond to the need for adjustments to individualised plans
  18. Support the person’s self-determination in making adjustments to plans
  19. Identify areas for improvement to overall service delivery implementation of organisation

How Is the Training Delivered?

Facilitate individual service planning and delivery course is available online and completed in your own time. This Facilitate individual service planning and delivery training course is based on the learning outcomes outlined above.

The course is typically designed to be completed in a minimum of four hours. However, the exact length of the course may depend on your ability and prior knowledge around the subject.

As the training is delivered online, you are able to complete it at your own pace.

What Accreditation Will You Get?

After completing the Facilitate individual service planning and delivery course, an assessment is conducted to determine if you have understood the information presented during the training course. If you pass this assessment, you receive a Certificate of Completion.

IMPORTANT NOTE whilst this course is based on the learning outcomes of the course provided, it is important to note that it is not a Nationally Recognised Statement of Attainment unless you opt for that option. Nationally Recognised training is only really required If you specifically require that unit of competency for a qualification or licence (typically this will require a practical component, further assessment and increased costs).

This course has been designed to comply with the best practices of national health and safety standards. Terminology has been chosen for clarity and ease of comprehension across jurisdictions. For example, ‘employer’ has been used to signify the broader term ‘PCBU’, while ‘WHS’ represents both work and occupational health and safety.

A Certificate of Completion is issued on successful completion of the course and you are required to get an 80% pass mark to be deemed as understanding the content. It is recommended that training is conducted every 2 years to stay refreshed.
