The Canadian French edition of "Services à domicile (SD) interRAI, manuel de l’utilisateur et formulaire d’évaluation" is translated from the English "interRAI Home Care (HC) Assessment Form and User's Manual." It has been designed to be a user-friendly, reliable, person-centered system that informs and guides comprehensive planning of care and services for elderly and disabled persons in community-based settings around the world. It focuses on the person's functioning and quality of life by assessing needs, strengths, and preferences. It also facilitates referrals when appropriate. When used on multiple occasions, it provides the basis for an outcome-based assessment of the person's response to care or services. The SD can be used to assess persons with chronic needs for care, as well as with post-acute care needs (e.g., after hospitalization or in a hospital-at-home situation).
This book contains introductory material, an item-by-item guide to completing the assessment form, the assessment form itself, and errata/addenda that align it with the Canadian French English edition, version 9.1.3. The Canadian French Edition has been customized for Canada, and the French language used in this publication is native to Canada.
About the Additional Materials
This book (published in 2021) includes additional materials that were not part of the previous edition (published 2012). These additional materials are an Errata and Addenda document that lists changes, updates, or additions to the published HC Canadian French manual and assessment form, version 9.1.2. These Errata and Addenda serve to align the HC Canadian French version 9.1.2 with the Canadian English version 9.1.3.
